Open Frame 1 Copy and paste as a new layer
You will need to resize this to suit
I Resized by 65%

Image - Rotate - Free rotate - 45 Degrees Right
.

Open up another paper of choice

I used paper 7

Copy and paste as a new layer, 

Move this layer below the frame layer.

Now with your eraser tool erase all the parts of this paper that you do 

not wanting showing.

Copy and Paste Element 1
Place to the left of your tag .or as you wish.
Use my finished tag as a guide
adjust size to suit.

Open Elements 34
place as you wish you may want to 
Re size these layer by 50% 
or see my tag as a guide for placement.I placed this element in the middle of the bottom of the frame

Open Elements
4 and place to the right side of your tag
resize again to suit, Once happy

Duplicate this layer - Image, Mirror,

Copy and paste elements
14, 27, 40, 42, 43 and 46

 and place at the at the bottom of your frame layer
you will need to adjust sizes to suit.
Place as you wish or as I have

Use my tag as a guide.

Copy and Paste your tube as a new layer
Place as you wish, Or as I have.I placed my tube above the frame layer
and deleted the base of the tube that i did not want showing.
Use my tag as a guide.
Delete any of the tube you do not wish to see on your tag.

Add any other names and copyrite information that is required,

Save as you would normally.
